The Road Map to Successful Transition is designed to serve as a guide for students transitioning from an Interim-Program School to public school or other education program. It was developed by the Nebraska Department of Education's SEAC Committee on Education of Students in Out-of-Home Placements to illustrate the primary steps and events which may occur during this process. The Road Map encourages self-advocacy, responsibility, commitment and accountability on the part of the student. It includes a list of records and forms typically required for school re-entry or enrollment as well as a section where the student can fill in contact information for officials at school and in the child welfare and juvenile justice systems.
