The sample Section 504 Plan was developed by the Mountain Plans Regional Resource Center and made available through the Utah State Office of Education’s “Section 504 and Americans with Disabilities Act Handbook”. It is meant to serve as a guide and resource for a school’s Student Assistance Team (SAT) in developing an individualized plan for a student eligible for educational services under Section 504 of the federal Rehabilitation Act of 1973.

When the school’s Student Assistance Team determines a student is eligible for services under Section 504, the SAT develops an individualized 504 Plan for that child or youth. The 504 Plan can be developed as part of a general education plan, but must be reviewed and updated at least annually.
