Individual Education Plan (IEP)
The sample Individual Education Plan (IEP) is based on the Student Record System (SRS) used by the majority of schools in Nebraska. It is intended to serve as a guide and resource for a school’s IEP Team when developing an individualized program for a student with a verified disability eligible for special education and related services under the federal Individuals with Disabilities Education Act (IDEA).

In accordance the Nebraska Department of Education’s “Rule 51: Regulations and Standards for Special Education Programs”, once a student is verified as having a disability under IDEA, the IEP Team develops an IEP for that child or youth. The student and the parents must be afforded the opportunity for input into the IEP, both during its initial development and periodic review.
