The Youth Court Questionnaire was developed by the Nebraska Foster Youth Council with assistance from the Supreme Court’s Through the Eyes of the Child Initiative. The purpose of the Youth Court Questionnaire is to provide a child or youth in the Child Welfare Court System with an opportunity to directly participate in their own case by sharing information about their current circumstances with the Juvenile Court Judge. This Questionnaire may be completed and submitted to the Juvenile Court by any child or youth with an open Child Protective Services (CPS) case, regardless of age or placement, at any time and as often as they would like. Because the circumstances of students in out-of-home care frequently change, these children and youth are encouraged to complete the Youth Court Questionnaire multiple times during the life of their case.

The Caregiver Information Form was developed by the Nebraska Supreme Court’s Commission on Children in the Courts to provide foster parents and other out-of-home caregivers the opportunity to provide information to the Juvenile Court Judge as per Nebraska Revised Statute §43-1314.02. Caregivers are encouraged to submit a completed Information Form prior to the child or youth’s case review and permanency hearings.
